From กลัว (gluua, to fear) +‎ ดอก (dɔ̀ɔk, flower) +‎ พิกุล (pí-gun, medlar) +‎ จะ (, auxiliary verb used to indicate futurity, likelihood, etc) +‎ ร่วง (rûuang, to fall; to drop); literally "to fear that a medlar flower would fall".

Derived from an old play about a princess called พิกุลทอง (pí-gun-tɔɔng; "Golden Medlar") who prefers to remain silent because whenever she speaks, a medlar flower will fall out of her mouth.
