This reflects some namespace inconsistency between Wiktionary and Wikipedia. Wiktionary has its own Image: namespace. [[Image:w:flute.jpg]], alas, yields the flute.jpg in the Wiktionary namespace, not the Wikipedia one. It's the same image, which is kind of confusing.

One solution is to just paste an actual URL from Wikipedia, thus:

http://www.wikipedia.org/upload/1/1a/Japan_coa.png

But that is really suboptimal, since the URL could change.
