Added to entries whose spelling isn't attested, and has been respelled to match Wiktionary's treatment or recent spelling standards of the language. It is to be placed right under the language header, or in cases of multiple etymology sections where some do not require the template, right under the appropriate Etymology N level 3 header.
Parameters

(optional): the language code of the entry.|from=
(optional): the original attested form from which the entry was normalized.|fromlang=
(optional): the language in which the original form is attested. Used in cases like Punic 𐤊𐤋𐤌 (klm), where the attested χουλούμ is actually in an Ancient Greek text, and will hence require this parameter to display the form in the correct settings (e.g. font).
