I think the etymology is incorrect. In Rugby League and Rugby Union the mouthguards are referred to as "dummies" and when a player "loses it" he will spit out his mouthguard to get more air so he can start fighting. This exact phenomenon is referred to be commentators as "spitting the dummy" in a quite literal sense.
