eǎdem (adj./pro.), not eādem (adv.)

eādem, with a long A, is an adverb: "by the same means," "likewise." Adjacent to mūtātǎ, this adverbial parse is unlikely, presenting a challenge to comprehension. It is more likely that eǎdem, short A, agrees with mūtātǎ, especially given the context of the motto's creation and likely meaning.

eǎdem mūtātǎ resurgo [the same]PRO.NOM.FEM.SG. [having been changed]PERF.PASS.PART.NOM.FEM.SG. [will rise up again]PRES.INDIC.1.SG

The implied subject of resurgō is presumably spīra or fōrma, FEM.SG.

Literal translation: "Having been changed, I rise up again the same." Loosely: "Although I've been changed, I rise up again the same (unchanged)."
