Is it okay to use talk pages as sandboxes? Here's my following guess:

Good job! Yes, for form 1 you need to know middle root consonants, middle vowels for perfective/imperfective and the verbal noun (maṣdar), which is also in the entry.

So, a more complete conjugation is:
